1. Log onto the website: www.obqsurvey.org
2. Enter username (yellow) and password (blue) exactly as is appears on piece of paper given to you by 4th period teacher.
3. Begin and complete survey all at once. Do not exit site until you are done. You cannot use username and password more than once.
